アンジェリーナ・ジョリーが両乳腺切除について書いた記事で目にした「inherited」の意味とは？
今週、両乳腺切除（double mastectomy）をしたことを告白して大きく話題になった、女優で監督のアンジェリーナ・ジョリー（Angelina Jolie）さん。

その発端となったニューヨーク・タイムズ紙の寄稿記事『My Medical Choice』を読んでみました。
すると、
Only a fraction of breast cancers result from an inherited gene mutation.
と書かれていました。
ポイント
“inherited” は「遺伝の」という意味です。
- an inherited disease or quality is one that is passed on through the genes
- inherited money or property has been given to someone when the previous owner died
（2. のようにお金や資産に使われると、「受け継いだ」という意味になります。）
「遺伝による遺伝子突然変異が原因の乳がんは、ごく一部に過ぎません」とアンジェリーナ・ジョリーは書いていたのでした。

冒頭、アンジェリーナ・ジョリーさんは自身の母親が10年もの闘病生活を経て、56歳でガンのため他界したことを告白。子どもに祖母のことを説明する時に自分にも同じことが起きないか聞かれ、子どもたちには安心するように伝えてきたものの、本当はアンジェリーナ・ジョリーさんにもその危険性があることを明らかにします。
彼女の BRCA1 と呼ばれる遺伝子に変異があり、乳がん（breast cancer）になる危険性は87%、卵巣がん（ovarian cancer）になる危険性は50%と医者に判断されたとのこと。BRCA1 に欠陥がある場合の乳がんにかかる確率は、一般的には65％だそうです。
Once I knew that this was my reality, I decided to be proactive and to minimize the risk as much I could. I made a decision to have a preventive double mastectomy. I started with the breasts, as my risk of breast cancer is higher than my risk of ovarian cancer, and the surgery is more complex.

But I am writing about it now because I hope that other women can benefit from my experience. Cancer is still a word that strikes fear into people’s hearts, producing a deep sense of powerlessness. But today it is possible to find out through a blood test whether you are highly susceptible to breast and ovarian cancer, and then take action.

I wanted to write this to tell other women that the decision to have a mastectomy was not easy. But it is one I am very happy that I made. My chances of developing breast cancer have dropped from 87 percent to under 5 percent. I can tell my children that they don’t need to fear they will lose me to breast cancer.

WHO（世界保健機関）によると、世界では毎年45万8千人の女性が乳がんでなくなっているそうです。また、アメリカでは BRCA1 や BRCA2 の検査にかかる費用は3000ドル以上で、多くの女性にとって障害となっています。
I choose not to keep my story private because there are many women who do not know that they might be living under the shadow of cancer. It is my hope that they, too, will be able to get gene tested, and that if they have a high risk they, too, will know that they have strong options.

米国立がん研究所（National Cancer Institute）は、BRCA 検査を受ける前に、以下のチェック項目を挙げています。
- 母親、娘、姉妹などの親族のうち、乳がんと診断された人が2人いる。そのうち一人は50歳以下で診断された（two first-degree relatives [mother, daughter or sister] diagnosed with breast cancer, one of whom was diagnosed at age 50 or younger;）
- 年齢に関係なく、母親、娘、姉妹または祖母や伯母などで、乳がんと診断された人が3人以上いる（three or more first-degree or second-degree [grandmother or aunt] relatives diagnosed with breast cancer regardless of their age at diagnosis;）
- 母親、娘、姉妹などの親族のうち、乳がんと卵巣がんを診断された人がいる。または、伯母や祖母などの親族のうち、乳がんと卵巣がんを診断された人がいる（a combination of first- and second-degree relatives diagnosed with breast cancer and ovarian cancer [one cancer type per person];）
- 母親、娘、姉妹などの親族のうち、両側乳がんと診断された人がいる（a first-degree relative with cancer diagnosed in both breasts [bilateral breast cancer];）
- 年齢に関係なく、母親、娘、姉妹または祖母や伯母などで、卵巣がんと診断された人が2人以上いる（a combination of two or more first- or second-degree relatives diagnosed with ovarian cancer regardless of age at diagnosis;）
- 母親、娘、姉妹または祖母や伯母などで、乳がんと卵巣がんの両方を診断された人がいる（a first- or second-degree relative diagnosed with both breast and ovarian cancer regardless of age at diagnosis; and）
- 男性の親族で乳がんと診断された人がいる（breast cancer diagnosed in a male relative.）

ABC の記事『Angelina Jolie’s Mastectomy: What You Should Know』によると、この中で検査をして陽性反応が出る人は0.125-0.25%だそう。でも、陽性反応が出ても、必ずしもアンジェリーナ・ジョリーと同じ選択を取らなければならないということはありません。状況に応じて手術以外の処置を取ることも十分考えられます。
このことは、アンジェリーナ・ジョリーの主治医であるクリスティ・ファンク（Kristi Funk）医師も病院のブログで述べています。
This blog describes the main stages of her treatment. It is important to emphasize that each woman’s case is different. Surgery will not necessarily be the right choice for everyone, and there are alternatives available. As Angelina says in her article, the important thing is to be aware of your options.
